Renewal of Registration :

  1. Duly filled Application Form for Renewal of Full Registration (Available in this office & also in Website).
  2. Fees. (See the chart of fees fixed by Council)
  3. Original BM&DC Full Registration Certificate; if registration certificate is laminated, a photocopy along with the laminated certificate.
  4. M.B.B.S./B.D.S. Original Certificate and photostat copy.
  5. Copy of Twelve digit “Taxpayer’s Identification Number” (e-TIN). (For Bangladeshi Doctor’s)
  6. Photocopy of Acknowledgement Receipt/Certificate of Return of Income (Latest Fiscal Year); for Bangladeshi Doctors.
  7. Photocopy of National Identity Card (NID Card).

* Photocopies must be attested by a Registered Physician/1st class Gazetted Officer with Seal, Signature and Date.

Renewal of Registration (MATS):

  1. Renewal Form (Available in this office & also in Website) to be duly filled up.
  2. Fees. (See the chart of fees fixed by Council)
  3. Original BM&DC Registration Certificate. If Registration Certificate laminated, a photocopy along with the laminated certificate.
  4. Photocopy of National Identity Card (NID Card).

* Photocopies must be attested by a Registered Physician/1st class Gazetted Officer with Seal, Signature and Date.
